Our rink sponsored skills sessions are conducted by the Woodbridge Community Center and The Club at Woodbridge to serve as an informational event for youth players interested in our programs. Clinics are not intended to serve as an evaluation or tryout and should not be construed as such.

I hereby certify that I am the legal parent or guardian and give my approval for the above named applicant to participate in the Skills Session programs I assume all risks inherent in and incidental to such participation and I hereby release, absolve, indemnify and hold harmless Township of Woodbridge, Woodbridge Community Center, The Club at Woodbridge its partners, officers, employees and staff, of any claim arising out of any injury to the applicant and that may require professional medical attention. I hereby authorize and request The Club at Woodbridge and the Woodbridge Community Center to act on my behalf under prevailing circumstances in the event of any injury, in the event that I cannot be reached in case of an emergency.
